If you prefer not to order on-line, via credit card, never fear. Follow the instructions below and you can order by mail using personal check or money order:
* Down load the Mail Order Form and print it.
* Calculate the appropriate Shipping & Handling to add to your order. To do this, just hit the "Buy Now From CCNow" button as if you were going to make an on-line purchase. Indicate the number of running logs in the quantity box. Indicate your shipping destination in the "Shipping to" menu. Hit the "Update" button. This will give you the correct shipping for your number of logs and destination. Use these amounts in making out your check or money order. Just exit the CCNow page.
* Make your check or money order payable to: "Patient Endurance Publications"
* U.S. Funds only, please. Thank you.
* Mail your form with payment to:

* Postage to Guam is the same as for any U.S. destination. Use normal First Class stamp & envelope.
Your order will ship from the West Coast as soon as your order and payment have been received. All orders ship USPS Priority Mail in the U.S. and APO destinations. You will receive an e-mail notice once your order has been received and again about 48 hours later, when it is shipped.
